4. That I am executing this affidavit to attest to the truth of the foregoing and to request from the Land Transportation Office of Naga City, a copy of said Certificate of Registration.

What is an affidavit of loss? According to (definitions.uslegal.com), Affidavit of loss is a legal document declaring the physical loss of a security through theft or destruction by fire/flood. It will contain all details regarding the loss, the owner's name and any information pertaining to the security, such as serial number or date of issue.
